About Us

Who we are?

Mosaic Seniors Care Society was incorporated in January 2021 to assist multi-ethnic seniors living in Metro Vancouver with their health care needs. We aim to advance health services to our seniors by providing, maintaining, and operating non-profit housing, assisted living and long term care residences. In addition to operating these residences, our goal is to utilize these settings as health care hubs for seniors where we run adult day programming, wellness programs and home health care services from. These programs will be run in a multi-ethnic and culturally sensitive manner for those seniors living within the residences and for those accessing services while living in their own homes.

At Mosaic Senior Care Society, our goal is to respect the dignity of our elders and provide them with meaningful programming and daily activities to meet their unique social and health care needs.

Current Facilities

Gateway Assisted Living

Gateway offers vulnerable seniors a safe home where they can lead a fully independent lifestyle and be supported with activities of daily living. Our facilities include 60 subsidized suites, each with a private entry and lockable doors.

Services:

  • 24-hour Certified Care Aide support with assistance in bathing and grooming
  • Three nutritional meals daily
  • Halal meal options available
  • Basic activity programs such as exercise, games, and educational in-service
  • Weekly housekeeping and linen laundry 24-hour emergency response system
  • Routine building maintenance

Who is Eligible? Adults living at Gateway Assisted Living, must be able to live independently and be cognitively well enough to make their own decisions, while requiring a supportive environment due to physical and functional health challenges.

What is the Cost? Gateway is a government subsidized home. Each resident’s monthly rate is set based on their level of income. The monthly rate charged is calculated by multiplying your after tax income by 70 per cent.

Location: Located just off of 13787 100 Avenue, Surrey BC, in the heart of Surrey’s City Centre, Gateway sits amongst some important amenities for our Muslim Residents.

How to Access :

  • Contact the Home and Community Care office in your Health Authority and request an assessment for service.
  • Healthcare professionals, such as a doctor, nurse, pharmacist or social worker, can also make a referral on your behalf.
  • If in the hospital, and you require help upon return, request an assessment for eligibility prior to returning home

  • Interview conducted by a health professional and eligibility assessment completed If eligible, recommendations are made for type of facility care

To access these publicly subsidized services at Gateway, residents must be qualified by the Fraser Health Authority.

Once operational, Mosaic Seniors Care Society will run adult day programs from its Gateway Assisted Living residence for multi-ethnic seniors.  The site is located at 13787 100 Avenue, Surrey, BC.

While activities vary with each program, the services offered to seniors may include:

  • Therapeutic recreation activities
  • Assistance with personal hygiene
  • Assistance with health care needs such as nursing assessments, monitoring chronic health conditions or medications
  • Social activities such as discussion groups, entertainment and crafts, as well as noon meals
  • Respite for family members and others to provide a break from their caregiving responsibilities

Once operational, Mosaic Seniors Care Society will schedule wellness programs from its Gateway Assisted Living residence for multi-ethnic seniors.  The site is located at 13787 100 Avenue, Surrey, BC.

While activities vary with each program, the services offered to seniors may include:

  • Nutritional management programs
  • Occupational therapist health assessments
  • Exercise management programs
  • Chronic health management programs
  • Healthy heart programs
  • Blood pressure monitoring
  • Health-related literature and programming

Provides professional privately paid (i.e. non-government funded) nursing care in your home and community. Home care nursing is for people who need care at home, temporarily or indefinitely, for a variety of health reasons. Home visits for nursing care occur seven days a week. Visits are scheduled in advance according to medical priority and where you live.

Mosaic Seniors Care Society will look to add home care services to our operations in the future.

Provides long term care for adults who can no longer live safely or independently at home due to their need for 24-hour nursing supervision in a secure environment, and their unpredictable or complex care needs which can no longer be managed in their home setting.

Additional services include personal care assistance, end-of-life care, meals, nutritional support, social work, and recreational activities.

Mosaic Seniors Care Society will look to add long term care services to our operations in the future.
